抑郁症和双相障碍疾病成本研究

Study of the costs of major depressive disorder and bipolar disorder

  • 摘要: 目的:测量抑郁症和双相障碍的直接成本和间接成本,探索疾病总成本的影响因素。方法:选取2019年5月至2021年7月铜陵市某二级甲等精神专科医院招募的抑郁症和双相障碍住院患者230例,收集患者过去一年内门诊及住院的医疗花销总费用和自费花费,以及所花费的非医疗费用作为直接成本,将患者因病无法工作的生产力损失,以及因病损坏财务的其他损失作为间接成本。从个人水平和社会水平上测算直接成本和间接成本。采用多重线性回归模型对疾病总成本影响因素进行分析。结果:个人层面抑郁症人均疾病总成本高于双相障碍,而社会层面,双相障碍人均疾病总成本远高于抑郁症。抑郁症和双相障碍的间接费用均高于直接费用。多重线性回归分析显示,工作状态、婚姻状况、收入水平以及疾病严重程度是影响疾病成本的关联因素。结论:抑郁症和双相障碍的经济负担较重,因精神障碍导致的生产力损失是疾病成本的主要部分。

     

    Abstract: Objective:To measure the direct and indirect costs of major depressive disorder (MDD) and bipolar disorder (BD), and to explore the drivers of the total costs. Methods:A total of 230 inpatients with MDD and BD recruited from a Seconary Grade A psychiatric hospital in Tongling City from May 2019 to July 2021 were selected. Inpatient's total outpatient and inpatient medical costs, out-of-pocket costs, and non-medical costs in the past year were collected as direct costs, and productivity loss caused by the patient's inability to work and other losses caused by damaging properties due to disease were defined as indirect costs. Direct and indirect costs were calculated at individual and societal levels. Multiple linear regression model was applied for detecting the influencing factors of disease costs. Results:The total cost of MDD was higher than that of BD at individual level, but the total cost of BD was higher than that of MDD at societal levels. The indirect costs of two mental disorders were higher than the direct costs. Multiple linear regression analysis showed that working condition, marital status, income, and disease severity were associated drivers of total costs. Conclusion:The economic burden of the two mental disorders is heavy, and the productivity loss due to mental disorders is the driving force of the disease cost.
