广义估计方程分析社会经济因素对玉林市学龄儿童恒牙龋的影响

Generalized estimating equation analysis of the influence of socioeconomic factors on permanent tooth caries among school children in Yulin City

  • 摘要: 目的:探讨社会经济因素与学龄儿童恒牙龋发病的关系。方法:基线募集玉林市946例7周岁儿童,问卷收集人口学、社会经济学、口腔健康行为、口腔健康服务利用等信息资料,检查口腔情况,每6个月复查一次,纵向调查持续1年。广义估计方程分析社会经济因素与7岁儿童恒牙龋1年发病情况的相关性。结果:1年后回访908例儿童,回访率为96.0%。第一恒磨牙龋病患病率随时间推移逐渐增加,第一恒磨牙龋失补牙面数(DMFS)增量(△DMFS)由310面递增至485面。广义估计方程分析的最终模型结果显示,母亲受教育程度低(95%CI:1.037~1.234,P=0.007)、父亲从事第二类、第三类职业(95%CI:1.017~1.188,P=0.024;95%CI:1.109~1.401,P<0.001)、家庭人均月收入低(95%CI:1.003~1.152,P=0.044)及的儿童第一恒磨牙△DMFS发生的风险较高。结论:母亲受教育程度较低、父亲职业地位以及家庭收入水平是学龄儿童恒牙龋发病的重要危险因素。

     

    Abstract: Objective: To explore the relationship between socioeconomic factors and the incidence of permanent tooth caries in school children. Methods: A baseline cohort of 946 seven-year-old children was recruited from Yulin City. A questionnaire was used to collect demographic, socioeconomic, oral health behavioral, and oral healthcare utilization data. Clinical oral examinations were performed, with follow-up assessments conducted every six months for 1 year. A generalized estimating equation(GEE) was employed to examine the association between socioeconomic factors and the 1-year incidence of permanent tooth caries in 7-year-old children. Results: At the one-year follow-up, 908 children were reassessed, yielding a follow-up rate of 96.0%. The incidence of caries in the first permanent molars increased over time, with the increment in decayed, missing, filled surfaces(△DMFS) rising from 310 to 485. The final GEE model indicated that children with low maternal education(95% CI=1.037-1.234, P=0.007), with fathers engaged in secondary or tertiary occupations(95% CI: 1.017-1.188, P=0.024; 95% CI: 1.109-1.401, P<0.001), and with lower per capita monthly household income(95% CI=1.003-1.152, P=0.044) had a higher risk of △DMFS increment in the first permanent molars. Conclusion: Low maternal education, paternal occupation status, and household income are significant risk factors for the incidence of permanent tooth caries in school children.
