前列腺癌组织细胞周期蛋白依赖性激酶2、7、9表达与临床病理特征及预后的关系研究

Relationship between the expression of cyclin-dependent kinase 2,7,9 in prostate cancer tissues and clinicopathological features as well as prognosis

  • 摘要: 目的:研究前列腺癌(PC)组织细胞周期蛋白依赖性激酶CDK2、CDK7、CDK9表达与患者临床病理特征及预后的关系。方法:选取2016年3月至2018年3月哈尔滨二四二医院收治的167例PC患者,术中收集癌组织及癌旁正常前列腺组织。应用免疫组织化学染色法检测CDK2、CDK7、CDK9蛋白表达情况,分析CDK2、CDK7、CDK9与PC临床病理特征的关系。随访5年,收集患者的生存情况资料,采用Kaplan-Meier分析CDK2、CDK7、CDK9阴性和阳性表达患者生存率情况,采用单因素和多因素Cox回归分析PC患者预后的影响因素。结果:与癌旁正常前列腺组织比较,PC组织CDK2、CDK7、CDK9阳性表达率升高(P<0.05)。有淋巴结转移、术前游离PSA含量越高、Gleaso评分越高、TNM分期越高,CDK2、CDK7、CDK9阳性表达率越高(P<0.05)。Kplan-Meier分析结果显示,CDK2、CDK7、CDK9阳性表达组生存率低于阴性表达组(P<0.05)。单因素Cox回归分析显示,术前游离PSA、Gleaso评分、淋巴结转移、TNM分期及CDK2、CDK7、CDK9表达情况与PC患者预后有关(P<0.05)。多因素Cox分析结果显示,术前游离PSA≥1.9 ng/mL、Gleaso评分≥7分、有淋巴结转移、TNM分期Ⅲ期、CDK2阳性表达、CDK7阳性表达、CDK9阳性表达均为PC预后不良的危险因素(P<0.05)。结论:CDK2、CDK7、CDK9在PC组织中呈高表达,其表达水平与PC患者术前游离PSA、Gleaso评分、淋巴结转移、TNM分期及预后相关。

     

    Abstract: Objective: To study the relationship between the expression of cyclin-dependent kinase(CDK) 2,CDK7, CDK9 in prostate cancer(PC) tissues and clinicopathological features as well as prognosis. Methods: A total of 167 PC patients admitted to Harbin 242 Hospital from March 2016 to March 2018 were selected, and cancer tissues and adjacent normal prostate tissues were collected during surgery. Immunohistochemical staining was used to detect the expression of CDK2, CDK7, and CDK9 proteins, and to analyze the relationship between CDK2, CDK7, CDK9 and clinical pathological characteristics of PC. Survival data of patients were collected during a 5-year follow-up, Kaplan-Meier was used to analyze the survival rates of CDK2, CDK7, CDK9 negative and positive expression patients, and univariate and multivariate Cox regression was used to analyze the factors influencing the prognosis of PC patients. Results: Compared with adjacent normal prostate tissue, the positive expression rates of CDK2, CDK7, and CDK9 in PC tissues were increased(P<0.05). The lymph node metastasis,elevated preoperative free PSA content, higher Gleaso score, and advanced TNM stage were associated with increased positive expression rates of CDK2, CDK7, and CDK9(P<0.05). The Kplan Meier analysis results showed that the survival rates of CDK2, CDK7, and CDK9 positive expression groups were lower than those of the negative expression groups(P<0.05). The univariate Cox regression analysis showed that preoperative free PSA, Gleaso score, lymph node metastasis, TNM staging, and expression of CDK2, CDK7, and CDK9 were correlated with the prognosis of PC patients(P<0.05). The multivariate Cox analysis results showed that preoperative free PSA≥1.9 ng/mL, Gleaso score≥7 points, lymph node metastasis, TNM stage Ⅲ, CDK2 positive expression, CDK7 positive expression, and CDK9 positive expression were risk factors for poor prognosis of PC(P<0.05). Conclusion: CDK2, CDK7 and CDK9 are highly expressed in PC tissues, and their expression levels are correlated with preoperative free PSA, Gleaso score, lymph node metastasis, TNM stage and prognosis of PC patients.
