柴胡疏肝散联合二甲双胍对2型糖尿病合并非酒精性脂肪肝大鼠的作用及机制研究

Effect and mechanism of Chaihu Shugan Powder combined with metformin on rats with type 2 diabetes mellitus complicated with non-alcoholic fatty liver disease

  • 摘要: 目的:探讨柴胡疏肝散与二甲双胍联合使用对2型糖尿病(T2DM)合并非酒精性脂肪肝(NAFLD)大鼠的作用效果,并探究其可能的作用机制。方法:随机选择10只作为正常对照(control)组,使用普通饲料进行喂养;通过饲喂高糖高脂饲料和静脉注射链脲佐菌素构建了T2DM合并NAFLD(T2DM-NAFLD)大鼠模型;40只T2DM-NAFLD大鼠随机分为模型组(model组)、柴胡疏肝散组(CHSG组)、二甲双胍组(MET组)、联合用药组(MET+CHSG组),每组10只。control组和model组大鼠用等体积生理盐水灌胃,持续14周。采用试剂盒检测血清和肝脏组织中白介素(IL)-6、肿瘤坏死因子-α(TNF-α)、血糖、胰岛素、瘦素、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、活性氧(ROS)、丙二醛(MDA)和抗氧化酶(SOD)的水平;采用苏木精—伊红染色和油红O染色检测肝组织病理损伤;采用实时荧光定量PCR(RT-qPCR)检测ACCFASSREBP-1cPPARα基因的表达;采用蛋白质免疫印迹分析检测肝组织中Nrf2/HO-1信号通路相关蛋白的表达。结果:与control组相比,model组大鼠的体重、血清HDL-C水平、PPARαLPL mRNA表达、肝脏组织GS和SOD活性、核Nrf2和HO-1蛋白表达水平均显著降低,而肝脏重量,血清空腹血糖、胰岛素、HOMA-IR、ALT、AST、TC和TG、IL-6、TNF-α水平,以及肝脏组织SREBP-1cACCFASIL-6TNF-αNF-κB表达,肝脏组织PEPCK和G6Pase活性,肝脏组织ROS和MDA水平均显著升高(P<0.05)。与model组相比,CHSG组、MET组、MET+CHSG组大鼠的体重、血清HDL-C水平、肝脏组织GS和SOD活性、核Nrf2和HO-1蛋白表达水平均显著升高,且MET+CHSG组显著高于CHSG组和MET组(P<0.05)。与model组相比,CHSG组、MET组、MET+CHSG组大鼠肝脏重量,血清空腹血糖、胰岛素、HOMA-IR、ALT、AST、TC和TG、IL-6、TNF-α水平,以及肝脏组织SREBP-1cACCFASIL-6TNF-αNF-κB mRNA表达水平,肝脏组织PEPCK和G6Pase活性,肝脏组织ROS和MDA水平均显著降低(P<0.05),且MET+CHSG组显著低于CHSG组和MET组(P<0.05)。结论:柴胡疏肝散联合二甲双胍对T2DM-NAFLD大鼠具有护肝作用,其机制可能与Nrf2/HO-1抗氧化信号通路有关。

     

    Abstract: Objective: To explore the effect of Chaihu Shugan Powder combined with metformin on rats with type 2 diabetes mellitus(T2DM) and non-alcoholic fatty liver disease(NAFLD), and to explore its possible mechanism. Methods: Ten rats were randomly selected as normal control(control) group and fed with ordinary diet. A rat model of T2DM complicated with NAFLD(T2DM-NAFLD) was constructed by feeding high-glucose and high-fat chow and intravenous injection of streptozotocin. Forty T2DM-NAFLD rats were randomly divided into a model group, a Chaihu Shugan Powder group(CHSG group), a metformin group(MET group), and a combination group(MET+CHSG group), with 10 rats in each group. The rats in the control group and the model group were gavaged with equal volumes of saline for 14weeks. A reagent kit was used to detect the levels of interleukin(IL)-6, tumor necrosis factor-α(TNF-α),blood glucose, insulin, leptin, triglyceride(TG), total cholesterol(TC), low density lipoprotein cholesterol(LDLC), high density lipoprotein cholesterol(HDL-C), aspartate aminotransferase(AST), alanine aminotransferase(ALT), reactive oxygen species(ROS), malondialdehyde(MDA) and antioxidant enzymes(SOD) in serum and liver tissue; hematoxylin and eosin(HE) staining and oil red O staining were used to detect liver histopathological damage; real-time fluorescence quantitative PCR(RT-q PCR) was used to detect the expression of acetyl coenzyme A carboxylase(ACC), fatty acid synthase(FAS), sterol regulatory element-binding protein-1c(SREBP-1c),and peroxisome proliferator-activated receptor alpha(PPARα) gene in liver tissue; western blotting analysis was used to detect the expression of proteins related to Nrf2/HO-1 signaling pathway in liver tissue. Results: Compared with the control group, the body weight, serum HDL-C levels, and PPARa and LPL mRNA expression, liver tissue GS and SOD activities, nuclear Nrf2 and HO-1 protein expression levels of the rats in the model group were significantly reduced, while liver weight, fasting blood glucose, insulin, HOMA-IR, ALT, AST, TC and TG,IL-6, and TNF-α, levels in serum, as well as the expression of liver tissue SREBP-1c, ACC, FAS, IL-6, TNF-α and NF-κB, liver tissue PEPCK and G6Pase activities, and the levels of ROS and MDA in liver tissue were significantly increased(P<0.05). Compared with the model group, the body weight, serum HDL-C levels, liver tissue GS and SOD activities, and nuclear Nrf2 and HO-1 protein expression levels of the CHSG group, MET group,and MET + CHSG group rats were significantly increased, and the MET + CHSG group was significantly higher than the CHSG group and MET group(P<0.05). Compared with the model group, the liver weight, serum fasting blood glucose, insulin, HOMA-IR, ALT, AST, TC and TG, IL-6, TNF-α levels of rats, as well as the levels of liver tissue SREBP-1c, ACC and FAS, IL-6, TNF-α and NF-κB mRNA expression, the activities of PEPCK and G6Pase in liver tissue, and the levels of ROS and MDA in liver tissue of the rats in the CHSG, MET, and MET+CHSG groups were significantly reduced(P<0.05), and the MET+CHSG group was significantly lower than the CHSG group and MET group(P<0.05). Conclusion: Chaihu Shugan Powder combined with metformin has a liver protective effect on T2DM NAFLD rats, and its mechanism may be related to the Nrf2/HO-1 antioxidant signaling pathway.
