Abstract:
Objective: To explore the clinicopathological features and prognosis of primary membranous ne-phropathy(PMN)in adults with hepatitis B virus(HBⅤ)infection.
Methods: The clinical data of adult PMN pa-tients admitted to the Department of Nephrology, The First Affiliated Hospital of Guangxi Medical University from January 2015 to October 2020 and confirmed by renal biopsy were retrospectively analyzed.The clinico-pathological features of patients with or without HBⅤinfection were compared, and the prognosis of the two groups was compared by Kaplan-Meier survival analysis and COX regression analysis.
Results: A total of 88 PMN patients were enrolled, including 49 patients with HBⅤinfection (PMN-HBⅤgroup) and 39 patients with-out HBⅤinfection (PMN group).The levels of aspartate aminotransferase (AST), alanine aminotransferase(ALT) and 24 h urinary protein in PMN-HBⅤgroup were higher than those in PMN group (
P< 0.05), while the complement levels of C3 and C4 were lower than those in PMN group(
P< 0.05).There were no signifi-cant differences in other clinical indicators and drug use between the two groups (
P> 0.05).Kaplan-Meier survival analysis showed that the clinical remission rate in PMN group was higher than that in PMN-HBⅤgroup(Log-rank
χ2=5.124,
P=0.024).Multivariate Cox regression analysis showed that co-infection with HBⅤwas the influencing factor for clinical remission of PMN(
HR=0.049, 95%
CI=0.214-0.899,
P=0.024).
Conclusion: Concomitant HBⅤinfection is a factor affecting the clinical remis-sion of adult PMN patients, and the prognosis of adult PMN patients with concomitant HBⅤinfection is poor.
